Purpose

To evaluate the accuracy of 4 intraocular lens (IOL) power calculation formulas for eyes that have undergone myopic laser vision correction (LVC) and to analyze the effect of incorporating posterior keratometry (PK) on prediction accuracy.

Setting

Single center, Republic of Korea

Methods

Four modern IOL power calculation formulas, with and without PK measurements, were evaluated using the Eyetemis online tool. Subgroup analyses were performed based on axial length (AL) as well as mean keratometry (K).

Results

In the total sample, median absolute SEQ-PE values ranged from 0.30 D to 0.42 D across formulas. The Barrett True K-PK and Hoffer QST-PK achieved the lowest median absolute SEQ-PE values (0.30 D and 0.31 D, respectively). Based on absolute SEQ-PE thresholds, 60% to 72% of eyes were within ±0.50 D and 88% to 94% were within ±1.00 D, with no statistically significant differences observed among formulas. Across all four subgroups, the EVO 2.0 formulas (either K or PK) achieved the lowest median absolute SEQ-PE values, ranging from 0.20 D to 0.36 D. Incorporating PK values resulted in significantly better outcomes in many comparisons, particularly in eyes with AL > 26 mm and mean K ≤ 40 D.

Conclusions

The BTK-PK, EVO 2.0-PK, and Hoffer QST-PK formulas demonstrated a trend toward higher accuracy in our study population, and incorporating PK values improved predictive accuracy, with more pronounced benefits in eyes with long AL and flat corneal curvature.
