Purpose
Impact of Swept-Source OCT Biometry on Refractive Predictability in Premium Intraocular Lens Implantation
Setting
To evaluate the impact of swept-source optical coherence tomography (SS-OCT) biometry on refractive predictability, visual performance, and patient satisfaction following implantation of premium intraocular lenses, specifically EDOF and trifocal IOLs.
Methods
This prospective study included patients undergoing cataract surgery with implantation of Vivity® or PanOptix® intraocular lenses. All eyes were measured preoperatively using swept-source OCT biometry, enabling accurate assessment of axial length, anterior chamber depth, lens thickness, and keratometry. Modern IOL calculation formulas were applied according to axial length and lens type. Postoperative outcomes included refractive accuracy, uncorrected distance visual acuity (UDVA), intermediate visual acuity (UIVA), near visual acuity (UNVA), residual astigmatism, and patient satisfaction.
Results
Overall, 72% of eyes achieved a postoperative spherical equivalent within ±0.25 D, while 94% were within ±0.50 D of the intended target refraction.
In the EDOF group, 92% of patients achieved UDVA ≥ 20/25, and 88% achieved UIVA ≥ 20/25, reflecting excellent distance and intermediate visual performance.
In the trifocal group, 90% of patients achieved UDVA ≥ 20/25, and 85% achieved UNVA ≥ 20/32, confirming effective near vision restoration.
Residual refractive astigmatism ≤0.50 D was observed in 91% of eyes. Overall patient satisfaction was high, with 89% of patients reporting satisfaction or high satisfaction, and a low enhancement rate (<5%).
Conclusions
wept-source OCT biometry provides the high level of precision required for premium intraocular lens implantation. Its use significantly improves refractive predictability and visual outcomes following EDOF and trifocal IOL implantation, supporting SS-OCT biometry as a cornerstone of modern premium cataract surgery.
