Purpose

To compare the refractive prediction accuracy of four intraocular lens (IOL) power calculation formulas (SRK/T, Barrett Universal II, Kane, and PEARL-DGS) in eyes undergoing Yamane intrascleral fixation of a posterior chamber IOL.

Setting

Retrospective, single-center study conducted at Başakşehir Çam and Sakura City Hospital, a tertiary referral ophthalmology center in Istanbul, Turkey.

Methods

This retrospective study included 51 eyes of 51 patients who underwent double-needle Yamane intrascleral IOL fixation and had complete biometric data for all four formulas and postoperative manifest refraction. For each eye, prediction error (PE) was defined as the difference between postoperative spherical equivalent and the predicted refraction; absolute error (AE) was the absolute value of PE. Mean error (ME), mean absolute error (MAE), median absolute error (MedAE), and the percentages of eyes within ±0.25 D, ±0.50 D, and ±1.00 D of target refraction were calculated. Inter-formula comparisons of AE were performed with the Friedman test followed by Bonferroni-corrected post-hoc pairwise analyses.

Results

The arithmetic ME ranged from –0.23±0.62 D (SRK/T) to +0.15±0.82 D (Kane). SRK/T yielded the lowest MAE (0.44±0.49 D), followed by Barrett (0.53±0.57 D), PEARL-DGS (0.54±0.56 D), and Kane (0.61±0.56 D). The proportion of eyes within ±0.50 D of target refraction was 72.5% for SRK/T, 60.8% for Barrett, 54.9% for PEARL-DGS, and 52.9% for Kane; ≥86% of eyes were within ±1.00 D with all formulas. Overall differences in AE among formulas were significant (p=0.017), but no pairwise comparison remained significant after Bonferroni correction.

Conclusions

All four formulas provided acceptable refractive outcomes for Yamane intrascleral IOL fixation, with more than 86% of eyes within ±1.00 D of target refraction. SRK/T showed the lowest MAE and the highest proportion of eyes within ±0.50 D, suggesting a slight numerical advantage, although inter-formula differences were modest and not statistically significant after adjustment for multiple comparisons.
