Purpose
To study the space between the posterior surface of IOL and the posterior capsule and its change over time for commonly used Hydrophobic IOLs implanted in the capsular bag
Setting
Dr Rajendra Prasad Centre for Ophthalmic Sciences, All India Institute of Medical Sciences, New Delhi
Methods
A comparative observational study was performed over 18 months enrolling 150 eyes with 5 different group of single piece hydrophobic in-the-bag IOLs (Acrysof IQ, Acrysof Panoptix, Clareon, Tecnis and Vivinex) for senile cataracts; 30 in each group. Eyes were evaluated preoperatively for routine parameters of cataract surgery and were evaluated at 1 week, 1 month & 3 month of post-operative period. Anterior segment ocular coherence tomography (AS-OCT) was performed to evaluate the space between posterior surface of IOL and PC. The quantity and quality of vision was also assessed.
Results
On day 1, Panoptix® exhibited the widest space (400.97 ± 169.30 µm) followed by Clareon® (366.87 ± 196.54 µm), Acrysof® IQ (354.53 ± 90.82 µm) and Vivinex™ (321.83 ± 152.96 µm), while Tecnis™ showed the smallest separation (144.97 ± 28.49 µm).
By three months, mean spaces converged to single-digit values for Tecnis™ (1.00 ± 4.03 µm), Vivinex™ (3.33 ± 3.79 µm), Panoptix® (4.53 ± 4.23 µm) and Clareon® (2.07 ± 2.23 µm), with Acrysof® IQ retaining slightly larger residual separation (26.33 ± 25.26 µm).
Total aberrations and HOAs were lowest with Vivinex™ (0.12 ± 0.02 µm) and Clareon® (0.19 ± 0.03 µm) and highest with Panoptix® (0.31 ± 0.05 µm).
No significant posterior capsular opacification was noted in any group in our period of follow up.
Conclusions
Refractive change does happen with the change in IOL position denoted by space between the back surface of IOL and posterior capsule.
IOLs associated with tighter late IOL-PC apposition also tend to show lower aberrations and superior unaided vision at three months.
