Purpose
To develop a modified method of RSP-III IOL implantation and to evaluate its clinical outcomes when implanted using an injector through a small self-sealing incision in cases of posterior capsule rupture during phacoemulsification or lens capsule insufficiency.
Setting
Group I (main): 38 patients (38 eyes) who underwent IOL implantation using the proposed technique.
Group II (control): 37 patients (37 eyes) who underwent IOL implantation with scleral suturing.
The main group included 38 patients (38 eyes): 18 men (47.4%) and 20 women (52.6%). The average age was 73.2 ± 5.2 years.
The control group included 37 patients (37 eyes): 18 men (48.6%) and 19 women (51.4%). The average age was 71.2 ± 6.1 years.
Methods
A new modified method of RSP-III IOL implantation was developed and patented in the State Register of Inventions of the Republic of Uzbekistan on August 18, 2016.
Results
The main group consisted of 38 patients (38 eyes) with a preoperative (preop) visual acuity of 0.04 ± 0.02 and a postoperative (postop) visual acuity of 0.7 ± 0.1. Preoperative astigmatism was 42.4 ± 0.25, and postoperative astigmatism was 42.4 ± 0.25. The IOL position was stable in 100% of cases.
The control group included 37 patients (37 eyes) with a preop visual acuity of 0.06 ± 0.05 and a postop visual acuity of 0.5 ± 0.1. Preoperative astigmatism was 42.5 ± 0.50, and postoperative astigmatism was 42.5 ± 0.50. Stable IOL positioning was achieved in 96% of cases.
Conclusions
- The proposed RSP-III IOL implantation technique using an injector is the method of choice in cases of posterior capsule rupture during phacoemulsification or failure of the lens ligament apparatus. This technique reduces surgical time, does not require special instrumentation, and does not demand advanced surgical skills.
- RSP-III IOL implantation using an injector allows for stable IOL position in the early and long-term follow-up periods, as well as high visual acuity with minimal corneal astigmatism.
