Purpose

To evaluate visual outcomes, task-specific intermediate vision, and patient satisfaction following bilateral implantation of the AcrySof IQ Vivity Extended Depth of Focus (EDOF) intraocular lens (IOL).

Setting

This prospective single-centre study included patients who underwent bilateral phacoemulsification with Vivity EDOF IOL.

Methods

Postoperative assessments at one month included uncorrected distance (UCDVA), intermediate (UIVA), and near visual acuity (UNVA), as well as responses to a modified Catquest-9SF questionnaire. Two additional items were included to assess difficulty in computer use and reading large-font text on mobile phones, tasks relevant to Vivity's intended optical benefits. Multivariable logistic regression was used to identify predictors of reported difficulty.

Results

A total of 170 patients (340 eyes) were enrolled. At one month, all patients achieved UCDVA and UIVA of 6/6, and 87% achieved UNVA of N8 or better. However, 26.3% of patients reported “Some Difficulty” with computer use or reading mobile phone text. Multivariable analysis identified shorter axial length (<23.3 mm) and worse preoperative visual acuity (<0.35 logMAR) as independent predictors of this difficulty. Visual disturbances were infrequent (occurring in ≤2% of patients), and 98.4% of patients reported overall satisfaction with their vision.

Conclusions

While the Vivity EDOF IOL delivers excellent distance and intermediate visual acuity with high patient satisfaction, a notable proportion of patients, especially those with shorter eyes or poorer baseline vision, may experience difficulty with intermediate digital tasks, though this requires confirmation with longer follow-up. These findings highlight the importance of tailored patient selection and task-specific outcome assessment in evaluating EDOF lens performance.
