Purpose
To evaluate the efficacy and safety profile Four Point Scleral Fixation Technique employing the Akreos AO60 and the Micropure Intraocular Lenses.
Efficacy end point. To compare visual acuity (VA) 3 months after scleral fixation in patients in whom the Akreos and Micropure IOL was used for aphakia management. Safety endpoint. To compare the procedure-related and post-surgical follow-up complications in both groups.
Setting
Anterior Segment Surgery Service at Asociacion Para Evitar la Ceguera en Mexico (APEC)
Methods
Single center, single-surgeon, retrospective, transversal and comparative study
Inclusion criteria:
- ≥ 40 yo, both genders, Aphakia
Exclusion criteria:
- Corneal opacity, Incomplete Medical Chart
- Ocular pathology that precluded an accurate refraction
Variables
- Post-operative uncorrected visual acuity (VA).
- Best corrected VA.
- Spherical equivalent deviation.
- Complications
Statitical analisis using Student t-test and Mann-Whitney for continuous ariables, Chi-squared test for categorical variables using the SPSS software (v.20) and the Prism software (v.8). A p-value <0.05 was considered statistically significant.
Results
70 patients were divided into two groups: 35 patients with Akreos AO60 and 35 patients using the Micropure Intraocular Lens
With an average follow up of 92 days. There were the following reported complications:
In the Akreos group 11.4% developed macular edema, 11.4% developed vitreous hemorrage, 2.85% developed choroidal detachement, 8.57% had increased IOP and 5.71% had IOL opacification
In the Micropure group 8.57% developed macular edema, 14.28% developed vitreous hemorrage, 2.85% developed choroidal detachement, 5.71% had increased IOP and none had IOL opacification
59.9 % of the included patients demonstrated less than one diopter Sph eq at the third-month follow-up visit. 20/30 or better visual acuity was evidenced for 46.6% of patients.
Conclusions
Refractive outcomes after secondary scleral fixation in aphakia patients with are favorable in eyes with good visual potential.
Refractive outcomes after GoreTex scleral fixation employing both IOLs are comparable, and both IOL platforms provided adequate refractive predictability.
Complications between groups are also comparable; nonetheless, the Micropure hidrophobic acrylic material prevents this IOL from opacification after gas tamponade after subsequent Retinal surgery
