Purpose

To assess visual performance, refractive accuracy, astigmatic correction, and patient-reported outcomes after implantation of the Clareon Vivity (Alcon), a non-diffractive wavefront-shaping extended-range IOL, in a consecutive clinical cohort of 597 eyes.

Setting

Retrospective consecutive cohort study conducted at Moorfields Eye Hospital (Private branch), London, United Kingdom, a tertiary cataract and refractive surgery centre.

Methods

All consecutive eyes implanted with the Clareon Vivity IOL between December 2023 and December 2024 were included. Primary outcomes were postoperative uncorrected and corrected distance visual acuity (UDVA, CDVA) and spherical equivalent (SE) measured 15–90 days postoperatively. Secondary outcomes included uncorrected and distance-corrected near visual acuity (UNVA, DCNVA), binocular visual acuity, refractive accuracy (proportion within ±0.50 D and ±1.00 D of target), and astigmatic vector analysis using Alpins methodology. Patient-reported outcomes were assessed using the Vision Correction Questionnaire (VCQ) in a subset.

Results

A total of 597 eyes were analysed. Mean postoperative UDVA and CDVA were 0.09 ± 0.14 and 0.01 ± 0.09 logMAR, respectively (both p<0.001 vs preoperative). Mean postoperative UNVA was 0.43 ± 0.20 logMAR and DCNVA 0.48 ± 0.18 logMAR, with a mean reading add of +1.53 ± 0.50 D. Mean binocular UDVA was 0.00 ± 0.08 logMAR and binocular UNVA 0.31 ± 0.21 logMAR. Mean postoperative SE was -0.29 ± 0.44 D; 84% and 96% of eyes were within ±0.50 D and ±1.00 D of target refraction, respectively. Cylinder decreased (-0.41 ± 0.33 D; p<0.001). Vector analysis demonstrated effective astigmatic correction (correction index 1.24 ± 0.60; 77% within ± 15°). VCQ data (n=65) indicated 91% spectacle independence for distance and 65% for intermediate tasks.

Conclusions

In this large consecutive cohort, implantation of the Clareon Vivity non-diffractive wavefront-shaping IOL resulted in excellent distance visual acuity, high refractive predictability, and functional near performance with strong binocular outcomes. Patient-reported findings were consistent with objective results. These data support its role as a predictable presbyopia-correcting option that extends functional range while maintaining refractive precision.
