Purpose

To compare the visual acuity and quality of vision after implantation of extended macular vision intraocular lens Vs standard monofocal intraocular lens in stable age-related macular degeneration patients with senile cataract

Setting

Prospective comparative pilot study at a tertiary eye care centre with all surgeries performed by a single surgeon.

Methods

Prospective comparative study of 20 eyes with stable age-related macular degeneration with central macular scar <1disc diameter undergoing phacoemulsification with implantation of extended macular vision IOL (10eyes) vs standard monofocal intraocular lens (10eyes). Primary outcomes measured were visual acuity and quality of vision (using aberrometry). Secondary outcomes were size of scotoma and occurrence of complications. Follow up was done at postoperative day 30 and 180.

Results

At 6 months, corrected and uncorrected visual acuity (near and distance) was comparable in both the groups (p>0.05). Total higher order aberrations were significantly less in extended macular vision lens implanted cases (group I- 0.186 ± 0.11µ, group II-0.822 ± 1.13 µ; p=0.015). Significant reduction in area of scotoma was observed in group I from preop value of 86.978 ± 19.37 mm2 to postop 84.718 ± 19.33 mm2 (p=0.004). No significant intraoperative or postoperative complications occurred.

Conclusions

Visual acuity in patients with extended macular vision intraocular lens and standard monofocal lens were comparable. Visual quality was found to be better in patients implanted with extended macular vision intraocular lens as compared with standard monofocal intraocular lens. Larger size sample and longer follow-ups are required to assess long term efficacy and safety.
