Purpose

The enVista Envy (Bausch + Lomb, USA) is a newly introduced hydrophobic acrylic trifocal intraocular lens designed to provide a full range of vision. Built on the established enVista platform, it features a proprietary diffractive optic intended to optimize light distribution across distance, intermediate, and near while maintaining contrast sensitivity and minimizing dysphotopsia. As one of the first early European adopters, we had the opportunity to explore its clinical potential in routine practice. Here we present the first European clinical experience and early visual outcomes following implantation of this new trifocal intraocular lens.

Setting

Single-site clinical practice at Gemini Eye Clinic in Zlin, Czech Republic

Methods

This case series included 13 patients (19 eyes) implanted with the enVista Envy trifocal IOL in routine clinical practice. One-month postoperative outcomes were analyzed. Evaluated parameters included uncorrected and corrected distance visual acuity (UDVA, CDVA), uncorrected and distance-corrected intermediate visual acuity (UIVA, DCIVA) at 66 cm, and uncorrected and distance-corrected near visual acuity (UNVA, DCNVA) at 40 cm, as well as subjective refraction. Monocular defocus curves were obtained to assess functional performance across a range of vergences. Photic phenomena, including halos and glare, were evaluated using a patient-reported questionnaire.

Results

At 1 month, all eyes achieved at least 0.2 logMAR across all distances, both uncorrected and distance-corrected. The mean UDVA was 0.06 ± 0.08 logMAR, and mean CDVA was -0.02 ± 0.07 logMAR. Intermediate at 66 cm and near visual acuity at 40 cm were mean UIVA: 0.02 ± 0.06 logMAR, mean DCIVA: 0.0 ± 0.06 logMAR; mean UNVA: 0.06 ± 0.07 logMAR and identical mean DCNVA:0.06 ± 0.07 logMAR. The monocular defocus curve demonstrated smooth performance across distance, intermediate, and near vergences with a cross of 0.2 logMAR between -3.0 and
-3.5 D. Reported halos and glare were generally mild and well tolerated so far.

Conclusions

Early European experience with the enVista Envy trifocal IOL at a single site demonstrates excellent visual outcomes at all distances, effective functional defocus performance, and a favorable photic phenomenon profile. These preliminary results support its potential as a presbyopia-correcting option in routine cataract and refractive lens exchange practice.
