Purpose

With my experience over 36 months with 164 eyes implanted with Panoptix multifocal ( mIOL)  and 200 eyes implanted with Alcon TORIC IOL , I decided tp implant Panoptix Toric for patients with borderline astigmatism wanting spectacle freedom and to judge the overall initial efficacy

Setting

Single centre, single surgeon, retrospective analysis . Centurion phaco machine ,Lenstar Biometry ( 2 technician) General Ophthalmology Clinic

No access to Verion /Callisto/ Pentacam /Wavefront Aberrometer/ Image Guided Cataract surgery suite

Methods

Strict patient selection as per multi focal IOL with routine surgery

Avoid patients with any ocular pathology

Avoid high myopes ( < 16D) uncertainty about the final ELP

Avoid high Astigmatism (>2 D) for the initial 30 eyes

Cross correlate with previous glasses / prescriptions

Post operative refraction done 1 week later

Results

Number of eyes 30/Bilateral implant 10 patients ( 20 eyes) /Unilateral 10 eyes

Male:Female 8:20/Age average 67.73 years

Phaco :Femto 16:14

Average K1/K2 difference 1.33 in PanToric

Average K1/ K2 difference in normal Toric 2.16

K1/K2 Panoptix difference 0.42( very low astigmatism)

Average IOL power in Panoptix Toric 23 D / Panoptix 22 D

Normal Toric

T3:11eyes & T4/T5 :89

T6T7:66 & T8/T9: 34

PanToric :T2 20/T3 10 eyes

Refractive Outcome

Panoptix Toric :24 eyes Plano 6/6, N6

6 eyes SE upto 0.25 D 6/6 p N6

Panoptix :Plano 144 eyes /SE 0.25D-0.5D 20 eyes

TORIC residual astigmatism :PLANO 80

0.25D-0.5D 71 eyes / 0.75D-1D 30eyes

1D-1.50D 17eyes/>1.5 D 2 eyes

Conclusions

In a general eye clinic with near basic equipment the surgeon has hand picked a careful limited section of patients who meet all the selection criteria

With a past experience in the TORIC IOL STUDY of 25% residual post operative astigmatism >0.5D  the surgeon implanted only cases with upto T3 Panoptix Toric as our centre did not have advanced technology equipment

With good post operative results 6 eyes having maximum 0.25D SE

24 eyes strictly Plano 6/6, N6

So careful patient selection , methodical screening with exclusion and cross correlation with past prescriptions/ glasses etc patient satisfaction can be expected
