Purpose

To evaluate functional visual outcomes, dysphotopsia, and patient satisfaction following bilateral implantation of a non-diffractive Full Depth-of-Field (Full-DoF) multifocal intraocular lens (mIOL) applied within a structured preoperative patient-selection protocol and standardized postoperative follow-up, in accordance with the ESCRS functional classification of presbyopia-correcting intraocular lenses.

Setting

Single-site private ophthalmic practice.

Methods

This retrospective observational study included 32 eyes of 16 patients undergoing bilateral implantation of a non-diffractive Full-DoF multifocal intraocular lens with a continuous defocus profile (RayOne Galaxy IOL). Twenty-six toric models were implanted. All procedures were performed by a single surgeon. Patient selection followed a structured protocol assessing visual needs and lifestyle, ocular surface and optical quality, binocular vision, and tolerance to visual disturbances. Postoperative evaluations at 1, 3, and 6 months included distance visual acuity, refractive outcomes, defocus curve analysis, patient-reported satisfaction, and dysphotopsia symptoms.

Results

Visual and refractive outcomes stabilized by 3 months and remained stable through 6 months. At 6 months, mean uncorrected distance visual acuity was 0.00 ± 0.05 logMAR and mean best-corrected distance visual acuity was −0.04 ± 0.08 logMAR. Approximately 85% of eyes were within ±0.50 D and over 95% within ±1.00 D of the intended spherical equivalent. Defocus curves demonstrated continuous functional vision from distance to near. Mild halo and glare were mainly reported at 1 month, decreased by 3 months, and were minimal at 6 months. No severe dysphotopsia or dissatisfaction was observed.

Conclusions

High and stable functional visual outcomes and patient satisfaction can be achieved with a non-diffractive Full-DoF multifocal intraocular lens when integrated into a structured patient-selection process and standardized postoperative management. The RayOne Galaxy IOL provides effective continuous-range vision with a low incidence of dysphotopsia when optical quality, binocular vision, and expectation management are systematically addressed. Postoperative follow-up supports neuroadaptation and long-term outcome stability.
