Purpose

To evaluate near and intermediate reading performance using an electronic reading desk after refractive lens exchange (RLE) with contralateral implantation of two diffractive trifocal IOLs, featuring identical add powers but distinct light distribution profiles.

Setting

International Vision Correction Research Centre (IVCRC), Department of Ophthalmology, University of Heidelberg, Heidelberg, Germany

Methods

Patients underwent RLE with contralateral implantation of Vivinex Gemetric (Hoya) in one eye and Vivinex Gemetric Plus (Hoya) in the fellow eye, irrespective of ocular dominance. Reading performance was assessed using Salzburg Reading Desk (SRD Vision) preoperatively and 6 months postoperatively. Binocular reading performance was assessed under uncorrected and distance-corrected conditions at fixed near (40 cm) and intermediate (80 cm) distances and at subjectively preferred near and intermediate distances. Twenty-nine patients are planned to be included in total.

Results

Six-month data from 19 patients showed improved binocular uncorrected reading acuity from 0.67±0.17 logMAR before the surgery to 0.17±0.12 logMAR after the surgery at 40 cm and from 0.56±0.23 logMAR to 0.12±0.10 logMAR at 80 cm. At subjectively preferred distances, postoperative uncorrected reading acuity was 0.08±0.11 logMAR at near (distance of 30.64±3.77 cm) and 0.13±0.11 at intermediate (63.74±8.59 cm). Binocular distance-corrected reading acuity showed a similar pattern, improving from 0.39 ± 0.15 logMAR to 0.17±0.12 logMAR at 40 cm and from 0.07 ± 0.08 logMAR to 0.12±0.10 logMAR at 80 cm.

Conclusions

RLE with paired implantation of Vivinex Gemetric and Gemetric Plus diffractive trifocal IOLs provided good postoperative reading acuity at near and intermediate distances, with overall improvement in reading performance. Near reading acuity at the subjectively preferred distance was slightly better than at the fixed 40 cm distance.
