Purpose
To evaluate visual outcomes, refractive predictability, binocular defocus curves, safety profile, and patient-reported outcome measures (PROMs) following bilateral implantation of the Tecnis OdysseyTM (Johnson & Johnson Surgical Vision®) intraocular lens (IOL).
Setting
Prospective, single-center, interventional study, conducted at the Department of Ophthalmology of Unidade Local de Saúde de São João (Porto, Portugal) between September 2025 and December 2025.
Methods
Nine consecutive patients underwent bilateral refractive lensectomy with implantation of the Tecnis OdysseyTM IOL. Monocular outcomes were analyzed per eye (n = 18), while binocular defocus curve analysis was performed per patient (n = 9). Distance visual acuity outcomes, including uncorrected (UDVA) and corrected distance visual acuity (CDVA), and refractive outcomes were recorded pre- and postoperatively (3 months). Binocular defocus curves were measured from −3.5 D to +1.5 D, and the area under the curve (AUC) was calculated for predefined functional ranges (distance, intermediate at 66 cm, near at 40 cm, and very near at 25 cm). PROMs were assessed using the validated Quality of Vision (QoV) questionnaire, scored on a 0–3 ordinal scale.
Results
Patients’ mean age was 56.6±4.5 years. Fifteen eyes (77.8%) received a spherical IOL. Mean UDVA improved from 0.443 ± 0.176 to 0.051 ± 0.078 logMAR (p<0.001). CDVA improved from 0.050 ± 0.054 to −0.009 ± 0.026 logMAR (p=0.006). Sixteen eyes (88.9%) achieved UDVA ≤0.1 logMAR. Mean postoperative spherical equivalent was -0.125 ± 0.177 D, with 16 eyes (88.9%) within ±0.25 D and 17 eyes (94.4%) with residual astigmatism ≤0.50 D. All patients presented a binocular depth of field from +0.50 to -3.50D, with visual acuities ≥0.2 logMAR. QoV results showed low overall symptomatic burden. Three patients reported frequent mild glare, two different patients reported frequent mild haloes, and one distinct patient reported frequent mild starbursts.
Conclusions
Bilateral implantation of the Tecnis OdysseyTM IOL resulted in significant improvements in uncorrected and corrected distance visual acuity and high refractive predictability. Binocular defocus curve analysis demonstrated a continuous full range of field profile with strong near and intermediate performance. PROMs showed low dysphotopsia burden, with absence of glare, haloes and starbursts in most cases. These findings suggest that the Tecnis OdysseyTM IOL provides effective functional vision across distances with minimal symptomatic compromise.
