Purpose

To evaluate the efficacy and patient satisfaction of a hydrophillic intraocular lens with a novel spiral design, in post refractive surgery patients who require spectacle independance.

Setting

Private Practice: Tygervalley Eye & Laser Clinic, Cape Town, South Africa

Methods

A series of patients who previously had corneal refractive surgery, with cataracts, were operated.
All patients required spectacle independance.
Femtosecond laser assisted cataract surgery was done in all cases and a spiral full range vision lens was inplanted in all cases.
In all cases there was no contraindication to implant a multifocal intraocular lens. Dry eye was managed in all cases prior to surgery. In some patients a custom match approach was used.

The visual results are compared to cases where refractive surgery had not been done prior to refractive cataract surgery.
The refractive outcomes and patient satisfaction results are discussed.

The Depth of Focus Curves are also presented.

Results

The visiual results at distance and near vision at 40cm, 50cm and 60cm are presented.

The results of spiral lenses bilaterally implanted are compared to a custom match approach where a enhanced monofocal lens of the same material was implanted in the dominant eye and the spiral lens in the non-dominant eye. This is an ongoing study and the results are contiually being updated.
Post LVC- Binocular uncorrected distance visual acuity was 20/16 in 75% of cases. 50% were 20/20 or better at 60cm and 100% were 20/25 or better at 50cm. 75% were 20/25 or better at 30cm

Conclusions

The novel spiral IOL was more forgiving than traditional trifocal lenses and less patietns were complaining of dysphohotopsia. With the custom match approach less haloes were experienced than in spiral lens cases. Spiral lenses placed in both eyes reding vision was better but distance vision less clear than when a custom match approach was used. In this series no lens exchanges were needed and patients were very happy.
