Purpose

Full visual range (FVR) intraocular lenses (IOLs) provide a continuous range of near, intermediate, and distance vision, offering patients a greater potential for spectacle independence. However, traditional FVR IOLs may deliver compromised visual quality compared with monofocal IOLs, such as reduced contrast sensitivity and a higher incidence of dysphotopsias. The enVista Envy™ (EN) and EN toric (ETN) FVR IOLs provide up to 4 diopters of continuous visual range and enhanced dysphotopsia tolerance. This retrospective case series study aimed to evaluate real-world visual and safety outcomes of the EN and ETN in patients undergoing cataract surgery.

Setting

This multicenter, cross-sectional, retrospective case series study includes data from 11 healthcare professionals from the USA (n=10) and Canada (n=1), covering the period from June 5, 2024, to September 3, 2025.

Methods

All data were extracted from standardized case report forms completed using electronic
medical records. Key outcomes included uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), uncorrected intermediate visual acuity (UIVA), uncorrected near visual acuity (UNVA), and refractive outcomes. Safety was also assessed.

Results

This analysis includes 166 eyes of 119 patients implanted with EN and 211 eyes of 147 patients implanted with ETN. Mean±standard deviation (SD) spherical equivalent target was −0.09±0.15 for EN and −0.07±0.17 for ETN. In eyes implanted with EN and ETN, respectively, 89% and 85% achieved a monocular CDVA of 20/20 or better, 92.5% and 91% achieved a monocular UDVA of 20/32 or better, 96.5% and 94% achieved a monocular UIVA of 20/32 or better, and 79% and 83% achieved a monocular UNVA of J2 or better. For EN and ETN, respectively, mean ± SD manifest refraction spherical equivalent was 0.07±0.39 D and 0.10±0.36 D; mean±SD residual cylinder was −0.35±0.30 D and −0.37±0.39 D. No serious adverse events were reported.

Conclusions

The findings of this case series demonstrate that the EN and ETN FVR IOLs deliver consistently strong visual and refractive outcomes across distance, intermediate, and near ranges for patients undergoing cataract surgery, with a favorable safety profile, supporting their use in real-world clinical practice.
