Purpose

To compare visual acuity outcomes, refractive accuracy, spectacle independence, and patient satisfaction between two trifocal intraocular lenses (IOLs): enVista Envy and PanOptix Pro.

Setting

Private practice refractive cataract surgery center.

Methods

This retrospective comparative case series included eyes undergoing cataract surgery or refractive lens exchange with implantation of either enVista Envy or PanOptix Pro trifocal IOLs. Postoperative surveys were administered at 1–3 months to assess spectacle independence and patient satisfaction. Clinical outcomes included uncorrected distance visual acuity (UDVA), uncorrected near visual acuity (UNVA), and average residual refractive error for best corrected visual acuity. Statistical comparisons between groups were performed using Fisher’s exact test.

Results

A total of 69 eyes implanted with enVista Envy and 98 eyes implanted with PanOptix Pro were included. UDVA of 20/20 or better was achieved in 81% of Envy eyes compared with 47% of PanOptix Pro eyes (p < 0.001). Mean near visual acuity was approximately J2 in both groups, with J1 achieved in 61% of Envy eyes and 46% of PanOptix Pro eyes. Refractive outcomes were excellent for both lenses, with an average residual refractive error of −0.14 ± 0.33 D for Envy and −0.16 ± 0.46 D for PanOptix Pro. 93% of Envy eyes and 83% of PanOptix Pro eyes were within ±0.50 D of target refraction. Full spectacle independence was reported by 83% of Envy patients and 81% of PanOptix Pro patients.

Conclusions

Both trifocal IOL platforms provided excellent refractive accuracy and high levels of spectacle independence. In this real-world cohort, enVista Envy demonstrated higher rates of 20/20 uncorrected distance visual acuity and tighter refractive predictability compared with PanOptix Pro.
