Purpose
To evaluate the visual, refractive, and patient-reported outcomes in cataract patients bilaterally implanted with an enhanced monofocal intraocular lens (IOL).
Setting
7 sites in the United States
Methods
This noninterventional, prospective, single-arm, multicenter study included 124 eyes from 62 patients who had undergone bilateral implantation with the enVista Aspire enhanced monofocal IOL (Bausch and Lomb). Both eyes were targeted for emmetropia (defined as ≤0.75 D manifest refraction spherical equivalent (MRSE)). Study parameters were manifest refraction, photopic monocular uncorrected distance visual acuity (UDVA), uncorrected intermediate (UIVA), binocular defocus curves, and patient-reported outcomes using Assessment of Intraocular Lens Implant Symptoms (AIOLIS) scores.
Results
At postoperative 60-270 days after the second IOL implantation, the mean MRSE was ‑0.24±0.30 D. The mean monocular UDVA and UIVA were 0.07±0.13 and 0.22±0.14 logMAR, respectively. UDVA of 20/32 or better was achieved in 91.1% of eyes, and UIVA of 20/40 or better in 80.6% of eyes. The frequency of glare, halos, and starbursts as ‘never to sometimes’ ranged from 93.5% to 100% of subjects; however, 91.9% to 98.4% were not at all’ to ‘a little’ bothered. More than 85% of patients could see pretty to perfectly well without glasses, and 93.5% rated their postoperative vision as good to excellent. The binocular defocus curve showed visual acuity of 0.2 logMAR or better up to a defocus of ~-1.25 D.
Conclusions
Bilateral implantation of enVista Aspire enhanced monofocal IOL resulted in excellent distance and good intermediate visual acuity, with a few visual symptoms and more than 90% of subjects expressing satisfaction with their postoperative vision. The good intermediate visual acuity found in the study is attributed to the IOL design. The enVista Aspire IOL features an intermediate 1.5-mm central zone with a blend zone between the central and peripheral powers on the posterior surface of the lens, designed to provide a gradual transition of incoming light, which enhances intermediate vision.
