Purpose
To assess early postoperative refractive and visual outcomes and to characterize the monocular and binocular defocus profiles after implantation of an enhanced monofocal intraocular lens ( XY1-EM Vivinex Impress, HOYA).
Setting
Athens Metropolitan Teaching Hospital (UNIC Athens), Athens, Greece.
Ophthalmo-Check Private Eye Centre
Methods
In this prospective/observational case series, 40 XY1-EM IOLs were implanted in 25 patients with cataract or dysfunctional lens syndrome; 15 patients received bilateral implantation. All IOLs were placed in the capsular bag. IOL power was calculated with the EVO 2.0 formula. No intraoperative complications were recorded. Monocular and binocular defocus curves were measured 1 month postoperatively using the Multifocal Lens Analyzer iPad application (Qvision).
Results
Mean postoperative spherical equivalent was +0.17 ± 0.24 D and mean refractive cylinder was −0.39 ± 0.36 D. At 1 month, the uncorrected monocular defocus curve showed good acuity across distance (0.02 logMAR), intermediate (0.22 logMAR), and near (0.47 logMAR) vergences. The binocular defocus curve demonstrated distance (−0.01 logMAR), intermediate (0.16 logMAR), and near (0.41 logMAR) performance. All participants reported functional spectacle independence for intermediate tasks.
Conclusions
At 1 month, capsular-bag implantation of the XY1-EM enhanced monofocal IOL yielded accurate refractive outcomes and provided good uncorrected distance vision with improved functional intermediate performance, as reflected by favorable monocular and binocular defocus profiles. Patient-reported intermediate spectacle independence was high. Larger studies with longer follow-up are warranted to confirm durability and broader patient-reported outcomes
