Purpose

To assess refractive results, binocular defocus profiles, visual phenomena, and the accuracy of intraocular lens (IOL) power calculations after implantation of the Clareon® monofocal (CNA0T0) IOL.

Setting

Department of Ophthalmology, Sveti Duh University Hospital, Zagreb, Croatia.

Methods

Prospective, single-arm clinical investigation. Consecutive patients without prior ocular surgery or clinically significant ocular pathology were enrolled. All eyes underwent uneventful phacoemulsification with implantation of the Clareon® monofocal intraocular lens (IOL). The primary outcomes were binocular distance and intermediate visual acuity. Secondary outcomes included refractive accuracy, binocular defocus profiles, visual phenomena, and the incidence of IOL glistenings. Postoperative assessments were conducted for up to 3 months.

Results

Fifty patients (90 eyes) were enrolled, and 44 (88 eyes) completed the three-month follow-up. Binocular defocus curves at three months indicated adequate intermediate visual performance, with mean visual acuities of 0.10 logMAR at −1.50 D (66 cm) and 0.30 logMAR at −2.50 D (40 cm). Dysphotopsias were rare; no intraocular lens glistenings were observed; and cystoid macular edema was detected in 3 eyes (3.4%) one month postoperatively. At 3 months, the mean spherical equivalent was −0.25 ± 0.38 D in myopic eyes, −0.09 ± 0.44 D in emmetropic eyes, and −0.10 ± 0.42 D in hyperopic eyes. Overall, 82%–85% of eyes achieved a postoperative refraction within ±0.50 D of the intended target, depending on the preoperatively defined refractive status.

Conclusions

Implantation of the Clareon® monofocal intraocular lens (IOL) resulted in excellent distant visual acuity, satisfactory functional intermediate vision, and high refractive accuracy with minimal dysphotopsia, absence of glistenings, and a low incidence of transient cystoid macular edema. The Barrett Universal II and Hoffer QST formulas demonstrated consistent refractive predictions across multiple refractive categories.
