Purpose

To report refractive and visual outcomes of patients in Hong Kong that underwent refractive lens exchange with bilateral Light Adjustable Lens.

Setting

Private-practice in Hong Kong.

Methods

Patients with significant error of refraction and completely dependent on glasses or contact lenses were evaluated for refractive lens exchange. Standard preoperative evaluation and biometry were performed. The LAL was implanted in the capsular bag and the eye was allowed to heal for approximately 4 weeks. When refraction was stable, trial lenses were used to determine target refraction for each eye, to achieve each patient’s visual goal. Up to 3 adjustments were performed until the patient was satisfied with their vision. Lock-in treatments were performed to prevent further refractive change of the LAL.

Results

A total of 8 patients, 16 eyes underwent bilateral refractive lens exchange. 4 were male. 6 with prior LASIK history. The average age was 54 ± 5.44 years old [range 47 -63]. 93.8% of eyes were within ± 0.5D of target MRSE. Preoperatively, 13.3% of eyes had ≤ 0.5D of keratometric astigmatism. After light treatments, 93.8% of eyes had residual manifest astigmatism ≤ 0.5D. Preoperatively, 12.5% of eyes had UDVA 20/20 or better. After light treatments, 87.5% of patients had binocular UDVA and UIVA of 20/20 or better, respectively. 90% of patients had binocular UNVA 20/25 or better.

Conclusions

The LAL is a viable solution for refractive lens exchange patients seeking spectacle independence. Refraction and vision significantly improved with adjustments. Customizing target refraction in each eye helps achieve individual visual goals and increase patient satisfaction.
