Purpose

To compare postoperative visual outcomes and optical quality between two extended depth-of-focus (EDOF) intraocular lenses (IOLs)—Eyecryl SERT and AcrySof IQ Vivity—highlighting the clinical importance of benchmarking two different hydrophobic EDOF optical strategies to optimize spectacle independence and patient satisfaction.

Setting

Prospective comparative clinical study conducted at a tertiary eye care center(Advanced Eye Centre, Postgraduate Institute of Medical Education and Research, Chandigarh, India) in patients undergoing standard phacoemulsification with in-the-bag implantation of an EDOF IOL.

Methods

Prospective comparative study of patients implanted with Eyecryl SERT EDOF (Biotech Vision Care Pvt. Ltd., Ahmedabad, India), (hydrophobic aspheric EDOF; >1.75 D depth of focus) or AcrySof IQ Vivity (Alcon Laboratories, USA)  (non-diffractive EDOF using X-WAVE™ wavefront-shaping technology). At 1 month, mean logMAR UDVA/CDVA (6 m), UIVA/CIVA (60 cm), UNVA/CNVA (40 cm), defocus curve (+1.00 to −4.00 D), HOA, MTF (modulation transfer function), mesopic contrast sensitivity (3–18 cpd), and visual function (VF) satisfaction scores were analyzed. p<0.05 was significant.

Results

Mean UDVA and CDVA were comparable between Eyecryl SERT and Vivity (p>0.05). Vivity showed significantly better mean UIVA, CIVA, UNVA and CNVA (p<0.05). At −3.00 D defocus, mean logMAR VA was 0.62 (Eyecryl SERT) vs 0.53 (Vivity). Total HOA were similar (p>0.05). MTF at 20 cpd was significantly higher with Vivity (p<0.05). Mean contrast sensitivity at 3 cpd (50.7 vs 59.7) and 6 cpd (98.4 vs 104.2) favored Vivity, while 12 and 18 cpd were comparable. Mean VF score was 91.95±SD (Eyecryl SERT) vs 96.20±SD (Vivity) (p=0.001).

Conclusions

Both IOLs provided excellent distance vision with comparable HOA. However, Vivity demonstrated superior intermediate and near acuity, better defocus performance, improved MTF at 20 cpd, enhanced low-frequency contrast sensitivity, and significantly higher patient VF satisfaction scores
