Purpose

To evaluate uncorrected distance, intermediate and near visual acuity and patient-reported dysphotopsia (type, severity and bothersomeness) after TECNIS PureSee™ extended depth of focus (EDOF) intraocular lens implantation in eyes with prior laser vision correction.

Setting

Retrospective chart review at a tertiary cataract and refractive surgery practice in Calgary, Alberta, Canada.

Methods

Retrospective chart review of adults implanted with TECNIS PureSee™ (toric/non-toric). Patients were classified as post–laser vision correction (LVC) if previous LASIK/PRK/refractive surgery was documented. Outcomes were extracted from postoperative visit day ≥28. Monocular uncorrected distance (UCDVA), intermediate (UCIVA) and near (UCNVA) visual acuity were recorded. Dysphotopsia (halos, glare, starburst) and bothersomeness were assessed; bothersome dysphotopsia was defined as moderate-to-severe bothersomeness. Visual acuity outcomes are reported per eye; dysphotopsia per patient. Follow-up completeness is reported. Outcomes are descriptive (proportions meeting Snellen thresholds).

Results

Nineteen patients (32 eyes) were included (mean age 64.4±5.7 years; 63.2% male). Toric PureSee was implanted in 19/32 eyes (59.4%). At postoperative day 28 or later, UCDVA was available in 31/32 eyes: 29/31 (93.5%) achieved ≥20/40, 24/31 (77.4%) ≥20/25 and 13/31 (41.9%) ≥20/20. UCIVA and UCNVA were available in 28/32 eyes each: UCIVA 26/28 (92.9%) achieved ≥20/40 and 16/28 (57.1%) ≥20/25; UCNVA 20/28 (71.4%) achieved ≥20/40 and 5/28 (17.9%) ≥20/25. Dysphotopsia assessment was available in 18/19 patients: bothersome dysphotopsia (moderate-to-severe) occurred in 2/18 (11.1%) (halos 3/18; glare 2/18; starburst 0/18).

Conclusions

In post-LVC eyes, TECNIS PureSee™ provided strong uncorrected distance and intermediate acuity with functional near acuity at ≥1 month, with a low rate of dysphotopsia that were not classified as bothersome. These findings support PureSee™ as a presbyopia-mitigating IOL option for post-refractive patients, a group in whom tolerance to residual refractive error and minimising photic phenomena are clinically important.
