Purpose

To evaluate the real-world visual outcomes and satisfaction of phacoemulsification and Vivity intraocular lens (IOL) implantation among Turkish/Mediterranean patients.

Setting

Yeditepe University, Medical Faculty, Department of Ophthalmology, Eye Center, Besiktas, Istanbul, Turkey.

Methods

In this retrospective study, phacoemulsification was performed in patients with bilateral senile cataracts, with implantation of a single-piece extended depth-of-focus (EDOF) intraocular lens (AcrySof IQ Vivity). The patient population was selected among patients requiring cataract surgery with no additional ocular pathology. Preoperative visit, one early postoperative evaluation within six months, and one late postoperative follow-up evaluation included uncorrected distance visual acuity (UDVA) and best-corrected visual acuity (BCVA) for near and far, slit-lamp examination, and fundus examination. Subjective parameters were assessed using the modified Catquest-9SF Questionnaire.

Results

172 eyes of 92 patients (53 males, 39 females) with a mean age of 60.94±9.7 years were included. Mean UDVA improved from 0.56±0.19 preoperatively to 0.92±0.11 at early follow-up and remained stable at 0.88±0.15 at late follow-up. Spectacle independence for near vision was achieved in 74 patients (80.4%), while the remaining patients required low-power additions (+0.75 to +1.50 D). Catquest-9SF scores (n=40) demonstrated high overall satisfaction, particularly for intermediate vision and night visual quality (p<0.001). Early complications affected 3 (1.74%) eyes (2 transient corneal edema, 1 IOL exchange due to IOL optic damage). One retinal tear was observed during the late follow-up period, managed with photocoagulation.

Conclusions

Bilateral implantation of AcrySof IQ Vivity provided favorable intermediate vision and distance visual acuity, achieving a high rate of complete spectacle independence (80.4%). Visual outcomes remained stable over a 2-year follow-up. Based on the Catquest-9SF questionnaire, patients reported high satisfaction scores, particularly in daily activities requiring intermediate focus and maintaining a high quality of vision even at night with minimal photic phenomena. These findings demonstrate that this IOL is a reliable surgical option for patients seeking reduced spectacle dependence with preserved visual quality.
