Purpose

To evaluate the effectivess of the Puresee IOL (Johnson & Johnson) in correcting presbyopia and any complications or side effects from using this lens.

Setting

All patients were treated at Kingsbridge private hospital Eye Unit in Belfast, UK.

Kingsbridge eye unit is a state of the art facility with dedicated refractive diagnostic and treatment optiions. It provides laser and lens based treatments for refractive patients.

Methods

- 21 consecutive eyes of 11 patients who received puresee IOL were analysed.

- All surgeriies were performed by the same surgeon (first author) who is qualified in refractive surgery.

- Patients had a full refractive assesment including monovision tolerance.

- Patients had a full discusion and consent regarding the available treatment options.

- All eyes operated had no other significant pathology and normal corrected vision.

- The dominant eye was targeted for emmetropia (1st minus) and the non dominat eye was targeted for -0.5 to -1.0

- Biometry was obtained using the IOL master 700 (TM). Standard K was used for the calculation.

- Post operative results were collected for refraction, vision in each eye, complications and any photopsia symptoms.

Results

- 10 patients had bilateral puresee IOL implant

- 1 had 1 eye for near vision only.

- There were no intra-operative or post opertive complications.

- All eyes targeted for distance achieved snellen 6/6 or better.

- 8 eyes (72.7%) targeted for near achieved N8 or better (range N5-N8)

- 3 eyes targeted for near achieved N10, but they all had a post op refraction of less than the minimum target of -0.5 (0 to -0.36 SEQ)

- From the 10 eyes targeted for distance, 5 also read N8, 1 read N6 and 1 read N5

- 2 patients reported some photopic symptoms at night but these were not deemed significant enough to impede their activities ncluding driving.

Conclusions

- The puresee IOL when implanted with slight monovision achieved excellent presbyopia correction when refractive targets are met.

- It was well tolerated by patients with no complaints regarding distance vision or significant dys/photopsia.

- In our cohort of patients; the achieved SEQ was often less than that targeted. This can be important when targeting reading vision. 3 of our patients achieved under the targeted SEQ and had slightly reduced reading vision. Intrestingly they were all myopes and so it maybe prudent to target a higher myopic target in this group.

- 70% of distance eys also achieved good reading vision with that eye. A larger study of this lens is required to help better refractive targeting and patient selection.
