Purpose
Premium intraocular lenses are traditionally avoided in eyes with macular pathology because of concerns regarding reduced contrast sensitivity and dysphotopsia, limiting postoperative spectacle independence. Extended depth-of-focus (EDOF) IOLs provide a continuous range of vision and may represent a viable option in selected vitreoretinal cases. This study evaluated anatomical and functional outcomes of EDOF IOL implantation during combined phaco-vitrectomy for full-thickness macular hole (FTMH).
Setting
M.M. Krasnov Research Institute of Eye Diseases, Moscow, Russian Federation
Methods
This prospective interventional study included 13 patients with FTMH not exceeding stage M according to the CLOSE classification and early cataract. All eyes underwent combined surgery: phacoemulsification with implantation of an EDOF IOL (Clareon Vivity, Alcon, USA) followed by standard three-port 25-gauge pars plana vitrectomy with application of autologous plasma to the macular hole. Balanced salt solution was used for endotamponade.
Best-corrected visual acuity (BCVA) was assessed preoperatively and 1 month postoperatively at distance (5 m), intermediate (66 cm), and near (33 cm) using ETDRS charts. Retinal morphology was evaluated with OCT.
Results
Macular hole closure with restoration of the foveal contour was achieved in 100% of eyes.
Mean BCVA improved significantly at all distances:
distance from 0.31 +/- 0.13 to 0.72 +/- 0.21,
intermediate from 0.35 +/- 0.10 to 0.68 +/- 0.28,
near from 0.34 +/- 0.07 to 0.53 +/- 0.32.
A statistically significant proportional correlation was found between distance and intermediate visual acuity gain.
Conclusions
EDOF IOL implantation during combined phaco-vitrectomy for FTMH provided favorable anatomical and functional outcomes with improved vision at all working distances. In selected patients with macular pathology, EDOF IOLs may allow a broader range of vision without compromising surgical results. Further studies with larger sample size and longer follow-up are required.
