Purpose

To outline the influences of postoperative pupil diameter on visual outcomes and satisfaction scores after implantation of a new fully refractive extended depth of focus intraocular lens (IOL).

Setting

Cathedral Eye Clinic, Belfast, Northern Ireland, United Kingdom.

Methods

The study enrolled 100 patients undergoing bilateral phacoemulsification with implantation of the Tecnis PureSee IOL (Johnson & Johnson). Postoperatively patients were categorized based upon their photopic pupil size: Group A: <2.50mm (n=13), Group B: 2.50 to 3.50mm (n=44), Group C: >3.50mm (n=23). Then categorized based upon postoperative scotopic pupil size: Group D: <4.50mm (n=30), Group E: 4.50 to 5.50mm (n=30), Group F: >5.50mm (n=18). Refraction, uncorrected (UDVA) and best-corrected (CDVA) distance visual acuities, uncorrected intermediate (UIVA) and near (UNVA) visual acuities and quality of vision (QoV) questionnaires were evaluated 3 months postoperatively.

Results

There was no significant difference between binocular UDVA, UIVA, UNVA and QoV scores between groups A. B and C.There was no significant difference between groups in postoperative spherical equivalent (SE) refraction in the dominant or nondominant eyes. In Group A, 79.9% reported spectacle independence for near, compared to 70.5% and 65.2% in groups B and C. There was no significant difference in binocular UDVA, UIVA, UNVA or SE across groups D, E and F. Group F showed significantly lower QoV than group D and E. Glare was reported by 20% in group D, 23.3% group E and 66.7% in Group F. Mean postoperative photopic and scotopic pupil sizes were 3.13±0.72 and 4.83±0.91mm, compared to 3.68±0.87 (P<0.001) and 4.90±0.89mm (P=0.14) preoperatively.

Conclusions

The Tecnis PureSee provides a wide range of vision, and high satisfaction. Postoperative pupil size appears to impact spectacle independence and scotopic pupil size appears to impact postoperative QoV and glare, however greater numbers would help investigate this relationship. At the conference we will present the outcomes of the same analysis with greater numbers.
