Purpose
To evaluate the 1-year clinical outcomes of the Tecnis® PureSee extended depth of focus (EDOF) intraocular lens and to assess its visual performance in comparison with the AcrySof® IQ Vivity intraocular lens.
Setting
This retrospective comparative study was conducted at Samsung Medical Center, a tertiary referral hospital in Seoul, South Korea. All surgeries were performed by experienced surgeons using a standardized surgical technique.
Methods
Patients who underwent cataract surgery with implantation of either Tecnis® PureSee or AcrySof® IQ Vivity EDOF intraocular lenses were included. Preoperative and postoperative examinations at 1, 3, 6, and 12 months included uncorrected and corrected distance visual acuity (UDVA and CDVA), manifest refraction, and refractive accuracy. Monocular uncorrected intermediate and near visual acuity were measured at 80 cm, 60 cm, 40 cm, and 33 cm. Defocus curves and contrast sensitivity tests were performed to evaluate functional visual performance. Patient-reported outcomes including spectacle dependence, incidence of dysphotopsia, and overall satisfaction were assessed using standardized questionnaires.
Results
A total of 65 eyes were included, with 25 eyes in the PureSee group and 40 eyes in the Vivity group. In the PureSee group, mean UDVA improved from 0.53 logMAR preoperatively to 0.07 logMAR at 12 months, and CDVA from 0.17 to 0.03 logMAR. At 12 months, UDVA and CDVA showed no significant differences between groups (p=0.642 and p=0.788). Intermediate visual acuity at 80 and 60 cm was comparable between groups. Defocus curve performance did not differ significantly at the original refractive target (approximately −0.25 D). With an additional −0.50 D myopic shift in PureSee, near visual acuity improved versus Vivity while maintaining intermediate vision. No significant differences were observed in dysphotopsia or satisfaction.
Conclusions
The Tecnis® PureSee EDOF intraocular lens demonstrated stable and favorable visual outcomes over a 1-year follow-up period. Compared with the AcrySof® IQ Vivity intraocular lens, PureSee provided comparable distance and intermediate vision. Applying an additional -0.50 D myopic shift improved near visual performance while maintaining comparable intermediate vision, supporting the clinical use of a PureSee mini-monovision strategy.
