Purpose
To evaluate visual outcomes and patient-reported results after bilateral femtosecond laser-assisted refractive lens exchange (RLE) with implantation of the non-diffractive extended depth-of-focus Clareon Vivity IOL (Alcon) using a mini-monovision approach in RLE patients.
Setting
University hospital.
Methods
In this prospective monocenter study, 44 eyes of 22 patients underwent bilateral femtosecond laser-assisted RLE with implantation of the Clareon Vivity. The target refraction was emmetropia in the dominant eye and mild myopia (approximately -0.75 D) in the non-dominant eye. At 3 months, postoperative assessments included binocular uncorrected (UDVA), target-corrected (TCDVA) and corrected (CDVA) distance visual acuity, uncorrected (UIVA), target-corrected (TCIVA) and distance-corrected (DCIVA) intermediate visual acuity at 66cm, uncorrected (UNVA), target-corrected (TCIVA) and distance-corrected (DCNVA) near visual acuity at 40cm, binocular target-corrected defocus curve and patient-reported spectacle-independence.
Results
The mean patient age was 59.6 ± 5.2 years and 45.5% (n=10) were female. At 3 months, mean binocular UDVA improved from 0.46 ± 0.32 logMAR preoperatively to -0.04 ± 0.08 logMAR, UIVA improved from 0.66 ± 0.27 to -0.02 ± 0.05 logMAR, and UNVA improved from 0.78 ± 0.25 to 0.14 ± 0.10 logMAR (all p<0.01, Wilcoxon test). Mean binocular CDVA was unchanged (-0.10 ± 0.07 vs -0.10 ± 0.06 logMAR, p=0.9), and no eyes lost 2 or more lines of CDVA. The binocular target-corrected defocus curve showed visual acuity of 0.10 logMAR or better between +0.5 D and -2.5 D. Spectacle independence increased across all tasks, with the highest rates for distance and intermediate vision.
Conclusions
Bilateral implantation of Clareon Vivity IOL using a mini-monovision approach resulted in good binocular visual acuity at far and intermediate distances, functional near vision, a wide range of defocus and high rates of spectacle independence, especially for distance and intermediate vision.
