Purpose

To evaluate visual performance and spectacle independence following implantation of the PURESEE (Johnson & Johnson Vision) enhance depth of focus (EDOF) intraocular lens, with subgroup comparison between diabetic and non-diabetic patients and between toric and non-toric IOL implantation, giving ongoing concerns regarding premium iol implantation in diabetics and refractively complex patients.

Setting

Setting:

Single-center private cataract surgery practice.

Design:

Tertiary eye care hospital

Retrospective observational consecutive multi-surgeon case series.

Methods

All consecutive eyes undergoing PURESEE implantation between February and October 2025 were included.  Minimum follow-up for visual assessment was 2 months.
Postoperative uncorrected distance vision acuity (UDVA) and uncorrected near visual acuity (UNVA) were analysed in 179 eyes . Primary endpoints were UDVA ≥6/9 and UNVA ≥N8. Proportions were reported with 95% confidence intervals. Subgroup comparisons were performed using chi-square or Fisher’s exact testing with statistical significance defined as p<0.05.
A structured telephone survey conducted ≥3 months postoperatively assessed spectacle dependence for intermediate (PC/mobile) and near (newspaper) activities using binary response recording survey response rate was 93.9%(168/179)

Results

Among 179 eyes, 97.2% achieved UDVA ≥6/9 (95% CI 93.6–98.8) and 35.2% achieved 6/6 or better. UNVA ≥N8 was achieved in 82.1% (95% CI 75.9–87.0), with 35.2% attaining N6 unaided.
Among eyes requiring postoperative near correction (n=153), mean near addition was +1.11 ± 0.36 D (median +1.00 D). 70.6% required ≤+1.00 D and 77.1% ≤+1.25 D. Near add requirement did not differ between toric and non-toric eyes (p=0.44) and was slightly lower in diabetic eyes (p=0.008).
A statistically significant difference was observed in UDVA ≥6/9 between diabetic and non-diabetic eyes (p=0.0009) and between toric and non-toric eyes (p=0.0078). No statistically significant differences were detected in near visual outcomes across subgroups (p>0.05).

Conclusions

In this consecutive real-world cohort, the PURESEE EDOF IOL demonstrated reproducible functional distance and near performance with high subjective satisfcation amongst patients. Although distance acuity thresholds varied statistically across certain subgroups, overall functional outcomes remained robust, including in diabetic and toric eyes.

Patient-reported spectacle independence for distance, intermediate and partial near tasks was high.
