Purpose

To evaluate real-world visual and refractive outcomes of a non-diffractive extended partial-range of field (PARTIAL-RoF) intraocular lens (IOL) in eyes with prior myopic LASIK undergoing cataract surgery or refractive lens exchange (RLE).

Setting

Tertiary referral university hospital.

Methods

In this retrospective, consecutive case series, 48 eyes of 33 patients with previous myopic LASIK who underwent cataract surgery or RLE with implantation of the non-diffractive Vivity IOL (Alcon) were included. Visual acuity outcomes included uncorrected (UDVA) and corrected distance visual acuity (CDVA) and uncorrected (UIVA) and distance-corrected intermediate visual acuity (DCIVA). Postoperative refractive outcomes and higher-order aberrations were analyzed. Linear mixed-effects models (LMM) were applied to evaluate associations between corneal aberrations and visual acuity while accounting for inter-eye correlation.

Results

Median postoperative CDVA improved significantly from 0.10 to 0.00 logMAR (p<.01). Postoperatively, 79.2% of eyes achieved CDVA ≤0.0 logMAR, and 75.0% achieved DCIVA ≤0.2 logMAR. Median postoperative UDVA and UIVA were 0.10 logMAR each. Manifest refractive astigmatism was ≤0.5 D in 79.2% of eyes. No significant association was found in the LMM between corneal spherical aberration or total higher-order aberrations and CDVA or DCIVA (all p>.05).

Conclusions

In post-myopic LASIK eyes, implantation of the non-diffractive extended PARTIAL-RoF IOL provided good distance and intermediate visual outcomes with tolerance to corneal aberrations. These findings support the use of non-diffractive PARTIAL-RoF IOLs as a viable option for presbyopia correction in this challenging patient population.
