Purpose
To evaluate visual performance, refractive accuracy, and rotational stability following bilateral implantation of a hydrophobic toric extended depth of focus intraocular lens (ELON IOL 877PETY, Medicontur Medical Engineering Ltd).
Setting
Prospective, single-centre clinical investigation.
Methods
This prospective, single-centre, single-arm interventional study enrolled adults undergoing bilateral cataract surgery with implantation of the ELON 877PETY toric EDOF intraocular lens, an aberration-neutral design extending depth of focus while correcting corneal astigmatism. Assessments at 1, 3, and 12 months included monocular and binocular visual acuity, manifest refraction, defocus curves, contrast sensitivity, and IOL rotational stability under photopic conditions. Patient-reported outcomes were evaluated using Catquest and PRISQ. The current interim analysis focuses on the 3-month outcomes.
Results
At 3 months, binocular DCVA was −0.05 ± 0.06 logMAR, demonstrating excellent distance vision, with functional intermediate vision (UIVA 0.24 ± 0.10; DCIVA 0.23 ± 0.09). Postoperative refraction was highly stable with SEQ tightly centered around emmetropia (97% within ±0.50 D and 100% within ±1.00 D SEQ), with postoperative cylinder clustering close to zero (92% ≤0.50 D; 97% ≤1.00 D). This demonstrates strong refractive predictability.Mean rotation was 2.90° between Day 1 and Month 1, indicating predictable toric alignment. Defocus curves are presented giving further insight into EDOF IOLs. Most patients experienced no or minimal difficulties with their daily activities post-implantation according to the patient reported outcome measures
Conclusions
This prospective clinical evaluation demonstrates that the hydrophobic toric EDOF IOL achieves excellent binocular distance acuity, functional intermediate performance, and high refractive precision with stable rotational alignment. Defocus analysis confirmed an extended functional range of vision, while patient-reported outcomes indicated low levels of visual difficulty. Twelve-month follow-up will to evaluate longer-term stability. This prospective study contributes meaningful clinical data to inform surgical decision-making regarding contemporary EDOF platforms in astigmatic cataract patients.
