Purpose

Trifocal IOLs may provide excellent visual acuity at all distance, but patients may experience bothersome dysphotopsia. In this study, we aim to evaluate the reading performance after binocular implantation of the Vivity IOL (Alcon, Fort Worth, TX, USA) combined with a mini-monovision approach using the Salzburg Reading Desk (SRD Vision).

Setting

International Vision Correction Research Centre (IVCRC), Department of Ophthalmology, University of Heidelberg, Heidelberg, Germany

Methods

In this study, 46 eyes of 23 patients have undergone bilateral delayed sequential femtosecond laser-assisted phacoemulsification and implantation of the AcrySof or Clareon Vivity IOL with mini-monovision of -0.75 D. Reading performance was evaluated preoperatively and at 6 months postoperatively. Monocular and binocular uncorrected and distance corrected reading acuity and reading speed were assessed at 40 cm and 80 cm distance and at the subjectively preferred near and intermediate distances.

Results

46 eyes of 23 patients with a mean age of 58 ± 5 years have been enrolled in the study. Before surgery, the mean binocular target-corrected reading acuity was 0,31 ± 0,16 logMAR at near (40 cm) and 0,09 ± 0,08 logMAR at intermediate (66 cm) distances. Six months postoperatively the mean binocular target-corrected reading acuity was 0,11 ± 0,07 logMAR at the set near distance (40 cm) and 0,09 ± 0,07 logMAR at the subjectively preferred near distance (39,97 ± 2,0 cm). At the set intermediate distance (66cm), the postoperative mean binocular target-corrected reading acuity was 0,07 ± 0,08 logMAR. The preferred intermediate distance was 58,9 ± 4,2 with a mean uncorrected reading acuity of 0,12 ± 0,07 logMAR. All Follow-Up visits were completed.

Conclusions

Intermediate and near reading function considerably improved after bilateral diffractive trifocal IOL implantation. The analysis shows a good reading function for the set and subjectively preferred distances.
