Purpose

To prospectively compare binocular visual acuity, defocus performance, and dysphotopsia profile of two contemporary non-diffractive extended depth-of-focus (EDOF) intraocular lenses (IOLs), Tecnis PureSee and Clareon Vivity.

Setting

Department of Ophthalmology, Koç University School of Medicine, Istanbul, Türkiye.

Methods

This prospective observational comparative study included 48 patients without significant corneal astigmatism (<1.0 D) or ocular comorbidity undergoing bilateral cataract surgery with Tecnis PureSee (n=28) or Clareon Vivity (n=20), implanted by a single experienced surgeon. At 6 months, binocular distance corrected distance (4 m), intermediate (66 cm), and near (40 cm) visual acuities (ETDRS) were assessed. Binocular defocus curves (+2.00 to −4.00 D, 0.50 D steps) were obtained. Functional defocus range was defined as visual acuity ≤0.20 logMAR. Contrast sensitivity, Quality of Vision, and Catquest-9SF questionnaires were evaluated. Statistical analysis was performed using t test or Wilcoxon test as appropriate (STATA 17.0).

Results

Both IOLs achieved excellent binocular distance vision (Vivity: −0.01±0.05 logMAR vs PureSee: 0.00±0.03 logMAR; p=0.213). Distance-corrected intermediate acuity had a mean of 0.07±0.09 logMAR for Vivity and 0.15±0.08 logMAR for PureSee (p=0.001), while distance-corrected near acuity was comparable (Vivity: 0.25±0.11 vs PureSee: 0.24±0.10 logMAR; p=0.704). Defocus curves differed at −1.0 D (Vivity: 0.02 ± 0.04 logMAR, PureSee: 0.07 ± 0.07 logMAR; p=0.039) and −1.5 D (Vivity: 0.06 ± 0.06 logMAR, PureSee: 0.13 ± 0.10 logMAR; p=0.015). Functional defocus range was similar with 3.34 D for Vivity and 3.38 D for PureSee. Glare was more frequent with Vivity (20% vs 7%; p=0.034). Contrast sensitivity and Catquest-9SF were comparable (p>0.05).

Conclusions

Both non-diffractive EDOF IOLs provided high visual performance at distance, intermediate, and near, with a similar functional defocus range. While Vivity seems to have slightly better visual acuity at intermediate distances, PureSee was associated with a slightly lower frequency of reported glare symptoms. Overall visual quality and patient-reported outcomes were comparable between the two lenses.
