Purpose

The aim of this retrospective study was to assess clarity 5 years after implantation of hydrophobic acrylic intraocular lenses (IOLs) containing hydroxyethyl methacrylate (HEMA).

Setting

Miyata Eye Clinic and Miyata Eye Hospital, Japan

Methods

Clinical records of 73 eyes of 73 participants who had participated in a previous 3-year randomized study involving implantation of Clareon IOLs containing HEMA (SY60WF, Alcon; group C) and hydrophobic acrylic Vivinex IOLs (XY1, Hoya; group V) and were followed up at 5 years postoperatively were reviewed. Corrected distance visual acuity (DCVA) and photopic and mesopic contrast sensitivity were evaluated. For clarity assessment, microvacuoles (MVs) due to glistenings and densitometry of the IOL central surface due to sub-surface nano-glistening were evaluated and compared between groups.

Results

There were 32 eyes in group C and 41 eyes in group V, and Nd:YAG laser posterior capsulotomy was performed in 4 and 5 eyes, respectively. The mean DCVA was −0.08 logMAR in both groups. There were no differences in contrast sensitivity at any spatial frequencies (P > 0.096). MVs were found in two and three eyes, respectively, and the numbers of MVs were three or fewer. The mean densitometry in group C (4.50%) was higher than that in group V (3.85%, P = 0.0082).

Conclusions

At 5 years postoperatively, there were no significant differences between the groups in Nd:YAG capsulotomy rates, DCVA, contrast sensitivity, and MVs, except for 16.9% higher densitometry in group C. This 5-year evaluation demonstrated the long-term clarity sustainability of Clareon IOLs containing HEMA.
