Purpose

To evaluate and describe hydrophilic intraocular lens (IOL) with opacification.

Setting

In our study we include 16 patients with opacification of hydrophilic intraocular lens that significantly impact their vision.

All IOL were explanted and examined by light microscopy (МТ8500 Meiji Techno, Japan), scanning electron microscopy, energy-dispersive X-ray spectroscopy (PEM-106I Selmi, Ukraine) and Alazarin Red staining

Methods

In our study, we included 16 patients with opacification of hydrophilic intraocular lenses that significantly impacted their vision.

All IOLs were explanted and examined by light microscopy (MT8500, Meiji Techno, Japan), scanning electron microscopy, energy-dispersive X-ray spectroscopy (PEM-106I, Selmi, Ukraine), and Alizarin Red staining.

Results

We identified five distinct patterns of IOL opacification.
Type 1 (tree growth ring): medium and large round lesions with density increasing from periphery to center, often located in the capsule-uncovered optic zone.
Type 2 (night starry sky): numerous small round deposits that progressively aggregate into larger confluent opacified areas.
Type 3 (mosaic): small square or polygonal lesions merging into a structured opacification zone.
Type 4 (frozen lava): a single extensive area of densely packed “cells” with sharp demarcation on high magnification.
Type 5 (fish caviar): dense central opacification with relative peripheral sparing and bright white cores.
All deposits consisted of Ca and P (3:1).

Conclusions

The main reason for hydrophilic intraocular lens opacification is the deposition of Ca and P in the affected zone.
