Purpose

To evaluate the safety, efficacy, and visual outcomes of retropupillary iris-fixated intraocular lens (IOL) implantation in aphakic eyes without adequate capsular support.

Setting

Tertiary eye care center / academic ophthalmology department.

Methods

A prospective/retrospective interventional case series including aphakic patients who underwent retropupillary iris-claw IOL implantation. Preoperative assessment included visual acuity, slit-lamp examination, intraocular pressure (IOP), and posterior segment evaluation. Surgery involved anterior or pars plana vitrectomy when indicated, followed by retropupillary enclavation of the iris-claw IOL. Postoperative follow-up assessed best-corrected visual acuity (BCVA), refractive outcomes, IOP, endothelial status, and complications.

Results

Significant improvement in postoperative BCVA was observed in the majority of eyes. Refractive outcomes were predictable and stable. Complications were generally mild and included transient IOP elevation, cystoid macular edema, pupil ovalization, and occasional disenclavation. No significant long-term endothelial decompensation was noted in most cases. The procedure demonstrated good anatomical stability and functional success.

Conclusions

Retropupillary iris-fixated IOL implantation is a safe, effective, and reproducible surgical option for secondary IOL implantation in aphakic eyes without capsular support, offering favorable visual outcomes with an acceptable complication profile.
