Purpose

To evaluate white-to-white corneal diameter measurments in a Saudi population using Pentacam and IOLMaster, assess inter-device agreement, and determine clinical implications for IOL sizing, refractive surgery planning, and anterior chamber depth (ACD) assessment.

Setting

A retrospective cohort study conducted at a specialized eye center in Riyadh, Saudi Arabia.

Methods

Medical records of 818 Saudi patients (18 years and above) with documented white-to-white (WTW) measurments between 2013 and 2023 were retrospectively reviwed at a specialized eye center in Riyadh, Saudi Arabia.

Bilateral WTW measurments were obtained using Pentacam and IOLMaster.

Keratometric parameters (K1, K2, Kmean, Kmax) and anterior chamber depth (ACD) were analyzed.

Independent and paired t-test, ANOVA, and Pearson correlation were performed to evaluate inter-device agreement and associations with age and sex.

Results

Mean WTW measured by Pentacam was 11.85±0.47 mm (right eye) and 11.85±0.44 mm (left eye), compared to 11.93±0.46 mm and 11.92±0.44 mm with IOLMaster. Strong inter-device correlation was observed (r=0.871, p<0.001).

Males demonstrated significantly larger WTW values than females (p<0.01).

WTW decreased significantly with age (p<0.001).

WTW correlated positively with ACD (r=0.512, p<0.001) and negatively with keratometric indices, suggesting biomechanical and anatomical interdependence.

Conclusions

WTW measurements showed strong agreement between Pentacam and IOLMaster in this Saudi cohort; however, significant age- and sex- related variations were identified. These findings highlight the importance of population-specific biometric references values when planning IOL sizing and refractive procedures. Incorporating demographic variability into surgical planning may improve refractive accuracy in Middle Eastern patients.
