Purpose
To evaluate surgical and refractive outcomes of iris-fixated intraocular lens implantation in pediatric eyes lacking capsular support and compare them with reported scleral fixation outcomes.
Setting
Tertiary referral center.
Methods
Five eyes of three pediatric patients (4–10 years) underwent iris-fixated IOL implantation for traumatic lens dislocation, aphakia without capsular support, and IOL dislocation. Postoperative outcomes included visual acuity, refractive predictability, IOL stability, and complications.
Results
All eyes demonstrated stable centration without tilt. Mean postoperative spherical equivalent was within ±0.75 D of target. Visual rehabilitation was successful in all cases relative to amblyopia risk. No retinal detachment, persistent uveitis, or chronic glaucoma occurred during follow-up. Surgical time was shorter compared to traditional scleral-fixation approaches.
Conclusions
Iris-fixated IOL implantation offers predictable refractive outcomes and excellent short-term stability in pediatric eyes without capsular support. Outcomes are comparable to reported Yamane scleral-fixated techniques, suggesting iris fixation is a safe alternative in selected pediatric cases.
