Purpose
To compare visual and refractive outcomes between primary intraocular lens implantation and aphakic correction in children under 5 years undergoing cataract surgery.
Setting
Tertiary pediatric ophthalmology unit.
Methods
Retrospective review of consecutive pediatric cataract cases under age 5. Patients were divided into primary IOL implantation versus aphakic correction groups. Outcomes evaluated included axial length growth, myopic shift, visual development, amblyopia therapy compliance, and need for secondary intervention.
Results
Primary IOL provided faster visual rehabilitation. However, greater myopic shift was observed in younger age groups. Aphakic children required intensive optical maintenance. Overall visual outcomes were comparable when strict amblyopia therapy was followed.
Conclusions
Primary IOL implantation in carefully selected pediatric patients offers practical advantages, though refractive predictability remains age-dependent. Individualized planning is critical.
