Purpose
To report an observed association between a specific pattern of anterior capsular fibrosis and chronic retinal detachment (RD) in eyes with mature cataracts. The aim is to highlight this finding as a potential clinical clue for preoperative prognostication, particularly in resource-limited settings.
Setting
Iclinix advanced eye care,New Delhi , India
Methods
A retrospective, observational case series of three patients who presented with mature cataracts was conducted. Preoperative assessment included slit-lamp examination, intraocular pressure (IOP) measurement, and B-scan ultrasonography. The key finding investigated was a pattern of fibrotic wrinkling on the anterior lens capsule.
Results
All three patients presented with mature cataracts and a distinct pattern of anterior capsular fibrosis. Intraocular pressure was normal in all cases, and there were no overt signs of hypotony. In this small cohort, B-scan ultrasonography confirmed a chronic, total retinal detachment in each eye, correlating with the anterior segment finding.
Conclusions
The presence of anterior capsular fibrosis may be a useful indicator of underlying chronic RD in eyes with mature cataracts. While this finding is not definitive, its presence could prompt a higher index of suspicion for posterior pathology. Further studies with a larger case series are required to establish the predictive value of this observation.
