Purpose

To evaluate the efficacy and safety of a novel surgical approach for hereditary lens subluxation, integrating CAPSULaser for continuous curvilinear capsulorhexis (CCC) with flanged 6-0 polypropylene suture for Cionni ring scleral fixation.

Setting

magrabi eye hospital, jeddah, ksa

Methods

Pre- and post-operative clinical data, including uncorrected visual acuity (UCVA), cycloplegic refraction, best-corrected visual acuity (BCVA), and intraocular pressure (IOP), were retrospectively collected from 16 eyes (mean age 12.3 ± 3.3 years) undergoing surgery by a single surgeon. Post-operative assessments included intraocular lens (IOL) centration, capsular bag stability, inflammation, and adverse events.

Results

Significant post-operative improvements were observed in mean refraction (pre-operative -11.33 ± 2.8D sphere, 6.2 ± 0.7D cylinder; post-operative -0.5 ± 0.7D sphere, -1.25 ± 0.9D cylinder) and BCVA (pre-operative 20/200; post-operative 20/25, p<0.05). Mean follow-up was 16.6 ± 3.1 months. All eyes achieved well-sized (5.1 ± 0.7 mm), centered CCC with a single laser shot and adequately centered IOLs. No significant IOP changes or procedure-related complications, including IOL malposition or conjunctival erosion of the flanged suture, were reported.

Conclusions

The combined CAPSULaser and flanged polypropylene suture fixation of Cionni ring offers a reproducible, safe, and effective solution for hereditary lens subluxation. This approach enhances intraoperative control, minimizes surgical time, reduces complication risks, and optimizes IOL stability and positioning, addressing prior surgical limitations.
