Purpose
Evaluating the initial experience and advantages of using selective
laser capsulotomy for capsulorhexis creation in cases of intumescent cataract.
Setting
magrabi eye hospital, jeddah, ksa
Methods
patients with intumescent cataracts scheduled for phacoemulsification
were included in this study. During surgery, after capsular stain with trypan
blue, the CAPSULaser device was used
to create a capsulotomy of size ranging from 5 to 5.2 mm and routine
phacoemulsification was done. The primary outcome measures were the
incidence of argentian flag sign, size, centration, and continuous capsulotomy
edge at the end of the surgery. Secondary outcomes were the surgical time
needed for capsulotomy together with capsulotomy centration over the optic of
intraocular lens (IOL) at 3 months, and any reported side effects related to the
treatment procedures.
Results
42 eyes of 42 patients were included, six of them were females. The mean
age of patients was 55±8.7 years. No cases of phacomorphic glaucoma included
in this study. Pre-operative IOP was 13±1.3 mmHg and post-operative was 12.1
± 1.55 that was statistically non-significant with no IOP spikes.
period 2.1 ± 0.7 months. The BCVA improved from HMGP to mean 0.02 ± 0.01
log MAR at end of follow‑up visits.. No cases of argentian flag signs, endothelial staining,
epithelial erosion, iris changes or phimosis were detected also in this study.
Endothelial cell count was 2333 ± 124 cells/mm pre-operative compared to 2274
± 122 cells/mm3 post-operative that was statistically non-significant.
Conclusions
Conclusion: CAPSULaser is reproducible, safe and effective tool in creating
well centered CCC with consistent 360 degree IOL coverage that ensures better
lens stability and effective lens positioning inside the capsular bags in cases of
intumescent cataract. This technique addresses previous limitations by
improving intraoperative control, minimize surgery time, reducing the risk of
well-known argentian flag sign and enhancing outcomes.
