Purpose

To evaluate visual and refractive outcomes following cataract extraction (cE) in eyes with prior implantable collaner lens (ICL ) implantation and to determine whether vault height serves as a predective factor for the timing of cataract development requiring surgical intervention.

Setting

It was a retrospective cohort study in tertiary centre in medical College .patients with prior implantable collaner lens implantation who subsequently developed cataract were included.vault height was measured by AS OCT .

Methods

This retrospective case series included eyes with previous phakic ICL implantation that subsequently underwent cataract extraction with IOL.preooerative evaluations were perform at 1 week,1 month,6 months including uncorrected visuls acuity UDVA ,corrected visual acuity CVA,manifest refraction.

Vault height measurements obtained early after ICL implantation and immediately prior to cataract extraction were analysed as continuous variables linear regression analysis were performed to assess the relationship between vault height and time interval to cataract extraction.refractive predictability and astigmatic outcomes were evaluated using standard vector and spherical equivalent analyses.

Results

At 6 months following cataract extraction,48 eyes were available for analysis .visual rehabilitation was favourable with mean efficacy and safety indices of 1.20+-1.11 and 1.50+-1.06 respectively demonstrating preservation and enhancement of visual function

Seventy percent of eyes achieved postoperative UDVA within one snellenmlinecof preoperative CDVA indicating high functional visual recovery.refractive outcomes demonstrated good predictability with 43% and 69% of eyes with +-0.25 D and +-0.5D of the intended spherical equivalent.

Astigmatic control improved postoperatively with the proportion of eyes demonstrating refractive cylinder <0.25 D and <0.50 D increasing ton40% and 60% respectively at 6months.

Conclusions

Cataract extraction following ICL implantation provides excellent visual rehabilitation with high refractive precision and favourable safety outcomes.vaukt height demonstrated a significant predictive relationship with the timings of cataract surgery supporting its role as a clinically meaningful biometric paramater in long term surveillance of ICL patients .

These findings reinforce that prior ICL implantation does not compromise visual recovery after cataract surgery and highlight vault height as a potential predictive biomarker for cataract progression offering clinicians a measurable parameter a measurable parameter to guide monitoring, councelling and surgical planning in this growing patients pololation.
