Purpose

Pain purpose is to introduce a simple and effective technique of phacoemulsification both for beginners and competent surgeons to avoid posterior capsular rupture intraoperatively.

Setting

Many techniques have been introduced and this one working with few modifications in steps of phacoemulsification brought a revolutionized change in our setup with good patient and ophthalmologist's satisfaction.

Methods

A prospecutive study with sample size of 1000 patients to introduce the benefits of whole nucleus chop in all types and stages of cataract including posterior subcapsular cataract 240 cases, nuclear sclerosis (grade 3) 700cases and mature cataracts 60 cases with age between 50 to 85 years 950 cases and 35 to 59 years 50 cases were operated.Exclusion only was subluxated crystalline lens.Modifications include hydrodissection, viscodissection to bring nucleus paryially above space between iris and anterior chamber not making it into pieces and emulsifying it directly with less use of secind instrument.Nucleus division or chopping into pieces is not done and there is no need of it.

Results

There is very less chance of posterior capsular rupture in this technique if there woild be any then the  nucleus can be revived through exrending the incision and there is quite less chance of it to be dropped posteriorly.Regarding endothelial safety, few measures were taken and the results were good with immediately clear cornea the next day of surgery and mark vision improvement.

Conclusions

No intraoperative complication such as iris injury, anterior capsular tear, zonular dehiscence or vitreous loss occured during surgeries. Most patients had 6/6 - 6/18 vision depending upon other underlying conditions of retina.
