Purpose

Pseudoexfoliation syndrome (PEX) can quickly turn a “routine” cataract case into a stressful experience for a beginner surgeon, not to mention the patient. Small pupils, fragile zonules, and capsular instability require mindfulness and anticipation rather than improvisation. This presentation shares ten practical survival tips designed to help novice surgeons stay calm, in control, and avoid preventable complications.

Setting

Educational surgical guidance derived from literature and tested in clinical cataract surgery practice.

Methods

A structured “Top Ten Survival Tips” approach is presented for ZEN SURGERY:

Zone of caution. Respect PEX from the start

Evaluate AC depth. Don’t go for the shallow ones

Nurture the pupil. If drugs don’t work, gentle use of hardware will

Stabilise AC.Use adequate CCI length and the right OVD to keep AC steady

Understand the circle. CCC should be appropriately sized and well-centred

Reduce phaco stress. No pushing, no rushing

Gentle but thorough hydrodissection. Let the fluid guide you

Evaluate IOL options carefully. Consider use of 3-piece IOL in the sulcus with reverse optic capture and CTR in the bag with severe zonulopathy

Remove cortex with tangential strokes only. Don’t yank it out

Yield if feeling unbalanced. Pause, breathe, and ask for support

Results

Applying ZEN SURGERY principles improves intraoperative control and surgeon confidence while reducing complications such as zonular dialysis capsular ruptures, vitreous loss, and late IOL decentration. A mindful, proactive rather than reactive strategy is key to maintaining capsular stability and inner peace, achieving predictable outcomes in PEX eyes in a young surgeon's hands.

Conclusions

In PEX eyes, routine thinking leads to complications. Anticipation, gentleness, and calm surgical flow are the beginner surgeon’s best allies. Mastering these ten survival tips transforms pseudoexfoliation from a feared case into a manageable surgical challenge.
