Purpose
To present a case of a 15 year old patient presenting with traumatic cataract with posterior capsular defect following blunt ocular trauma and to describe the clinical presentation, surgical approach and post-operative visual recovery
Setting
Tertiary eye care centre in Southern India
Report of case
A 15-year-old child presented with decreased vision following blunt ocular trauma. Clinical examination revealed a Mature Traumatic cataract with evidence of Posterior capsular rupture and lens matter in Anterior vitreous Face.AS-OCT showed fluid pockets in Cataractous lens with Posterior capsular defect. Posterior segment evaluation was limited due to cataract and B-scan showed no significant Vitreous or retinal pathology. The Patient underwent Lens aspiration and PCIOL placement in sulcus through Limbal approach combined with pars plana removal of the lens matter in the anterior vitreous face. Pre-operative, Intra-operative and Post-operative findings will be discussed.
Conclusion/Take home message
Traumatic cataract with posterior capsular rupture in paediatric patients presents unique surgical challenges. Lens aspiration and PCIOL implantation through limbal approach combined with Pars plana Vitrectomy provides effective anatomical and functional rehabilitation when carefully planned and executed. Early surgical intervention in these cases is critical in optimizing visual outcomes.
