Purpose

To evaluate visual outcomes and patient satisfaction following bilateral implantation of a non-diffractive spiral optic intraocular lens in a patient with Fuchs dystrophy and angle closure after refractive lens exchange.

Setting

Cornea and refractive surgery department of Quironsalud Hospital, Marbella, Spain.

Report of case

Methods

This is a prospective case report of a 45 years-old patient with Fuchs endothelial dystrophy and angle closure who underwent bilateral refractive lens exchange followed by implantation of a non-diffractive spiral optic intraocular lens. Intervention included standard phacoemulsification with IOL implantation. Outcome measures included uncorrected and corrected visual acuity, contrast sensitivity, patient-reported satisfaction, and quality of vision assessed at follow-up visits with VF-14 quiestionaire.

Results

Postoperatively, uncorrected distance visual acuity was 20/15 in both eyes, with uncorrected intermediate and near visual acuity of 20/20. The VF-14 questionnaire demonstrated high functional scores, with the patient reporting excellent quality of vision and no halos or night-vision disturbances. Overall satisfaction was maximal, and outcomes were statistically significant compared with the preoperative baseline.

Conclusion/Take home message

Conclusion

Bilateral implantation of a non-diffractive spiral optic IOL after refractive lens exchange provided excellent visual acuity, high functional performance, and patient satisfaction in a Fuchs dystrophy patient with angle closure, supporting its safety and effectiveness in complex cases.
