Purpose
to show importance of doing vortex vein decompression surgery in nonophthalmic eyes in controlling IOP and reducing choroidal effusion which lead to reduce the risk of complications that may occur during phaco surgery in nanophthalmic eyes
Setting
The Memorial Institute for Ophthalmic Research
Report of case
37 years old patient ,Bil. drop of vision
Examination showed that the patient had bilateral nanophthalmos
Bilateral axial lens 16.5 mm
Examination of both eyes:
Very faint cataract
very high hyperopia+18.00
UCVA 1/60 , BCVA 6/60
Congested episcleral vessels
High IOP Rt 35 Lt 38 (wit out ttt)
On full antiglaucoma ttt Rt 20 and LT 25
Rt Crowded disc with macular thickening
Lt cupping with C/D ratio 0.5 with macular thickening
Due to:
- very high IOP
- sever choroidal thickening
- very faint cataract
So, I decided to do vortex vein decompression before cataract to avoid complications during surgery and control IOP
steps of surgery ( shown in video)
6 months Post operative:
IOP 12 mmHg ( only 1 antiglaucoma)
VA 6/60 (the same)
Mild improvement in the choroidal thickening (2.63mm to 2.50 mm)
Marked improvement in the episcleral congestion (as shown in pic.)
After 6 months , we did the phaco in the original way
Conclusion/Take home message
vortex vein decompression surgery play an important role to prevent complications in nanophthalmic eyes
