Purpose
The range of care in cataract surgery continues to expand with the arrival of the latest generations of hydrophobic trifocal implants. This study aims to evaluate the refractive and clinical outcomes in patients who received the preloaded C-loop intraocular lens, AT ELANA® 841P (Carl Zeiss Meditec AG, Jena, Germany), in the context of cataract surgery or refractive lens exchange, by analyzing the visual and refractive performance obtaines after implantation.
Setting
Between April 2025 and today, a retrospective and prospective study was conducted within the OPHTALEA Group. FIfty-one patients (n=87 eyes) received ZEISS AT ELANA 841P intraocular lenses, either during cataract surgery or refractive lens exchange. This was a multicenter, non-comparative study involving two surgeons.
Report of case
The main exclusion criteria were : histoy of ocular surgery or traum, conditions affecting visual acuity or visual field (including maculopathies), psychological profile incompatible with a multifocal implant, corneal astigmatism of least 1.0D, and a kappa angle greater than 0,5. Each patient underwent a comprehensive preoperative assessment, including measurement with the ZEISS IOLMaster 700 an IOL power calculation using the Barett Universal II formula directly on the sweptsource biometer. Evaluation criteria at 1 month and 6 months included uncorrected visual acuity at various distances (far, intermediate, and near), residual astgmatism, residual spherical equivalent, and the VF14 visual quality questionnaire score. An anatomical check was also performed between Day 1 and Day 7.
One month after surgery, the mean residual spherical equivalent was +0,11 D with a mean residual astigmatism of 0,2 D, and 100% of eyes showed a spherical equivalent below 0.5 D. Mean uncorrected distance visual acuity reached most of 20/25, with significant improvement compared to the preoperative period (p < 10-6 ). All patients reported satisfaction with postoperative visual comfort, with VF14 scores ranging from 51 to 56. The visual performance observed at 1 month was maintained at 6 months (p > 0.05).
Conclusion/Take home message
The results of this study demonstrate that the ZEISS AT ELANA 841P intraocular lens provides excellent visual performances, ensuring satisfactory control of both spherical equivalent and residual astigmatism.
